Responsibilities:

• To manage caseload and provide support for patients and family caregivers:

  • To carry out assessment for patients and caregivers and devise plan of intervention and support and mobilise resources to aid their coping.

  • To carry out telephone counselling and outreach/face-to-face visits (home, hospital, centre, etc.) and run wellness / self-empowerment programmes for patients and caregivers.

• To coordinate and organise competence training programmes for professionals, staff, volunteers and home care workers, etc. both online and physically.

• To work on the learning materials or manual useful in the programmes.

• To promote the programmes to local hospitals, district centres, health clinics, patient groups, NGOs, elderly homes, government bodies and the general public in the community.

• To supervise and train junior staff and volunteers.

• To carry out duties as assigned by the supervisor.

Requirements:

• Degree holder in Social Work or related disciplines.

• Registered Social Worker by the Hong Kong Social Workers Registration Board of Hong Kong.

• Experience in working with cancer patients will be an advantage.

• Sound communication skills, efficient organising & liaison abilities and good report writing skills.
