Key Responsibilities:

• Supervise Care Coordinators and Registered Nurses in delivering care coordination and nursing services;

• Oversee the provision of health assessments, disease and complication screenings, nurse clinics, patient empowerment programs, and health promotion and prevention initiatives;

• Plan and manage manpower and resource allocation to support center operations and District Health Centre (DHC) primary healthcare services;

• Coordinate with multidisciplinary teams to ensure effective care coordination and bi-directional referrals between the DHC and community network service providers;

• Lead the implementation planning of new service initiatives.

A Bachelor’s Degree in Nursing;

• Holder of valid Registered Nurse practicing certificate issued by the Nursing Council of Hong Kong;

• With 10 years or above of clinical experience as Registered Nurse;

• Post-registration training in DM care/ Cardiology / Primary Health Care/ Community nursing / Orthopedic care / Gerontology / Gynecology would be an advantage;

• Passionate in community primary healthcare service and with strong sense of commitment;

• Proficiency in managing multidisciplinary team, with strong interpersonal, coordination and organizational skills.

• Strong command of both written and spoken English and Chinese;

• Proficiency in PC applications such as Microsoft Word, Excel, and Teams;

• Self-motivated, detail-oriented, responsible, and trustworthy, with strong interpersonal skills;

• Effective communication and organizational abilities.
